　Keith-Wagener分類（以下K-W分類と略す）第Ⅳ群即ち狭義悪性高血圧症の予後は，報告者の如何に拘らず，極めて不良であつて，Wage—ner等（1939）は，5年間死亡率を99％と記し，その後ほぼ同様の規準によつて調査したPalmer等（1948），Frant等（1950），Smithwick （1951）の成績も，それぞれ90％，100％，100％，の死亡率を掲げている。また，このことは，われわれの日常の経験とも一致しており，特に再検討の必要が認められないほどである。

　これに反して，眼科医が日常最も多く遭遇する良性高血圧の眼底変化，即ちThiel, Amsler等のいう高血圧性眼底（Fundus hypertonicus），K-WⅠ群とⅡ群に相当する眼底所見をもつ症例の予後に関しては，報告者によつて死亡率に著しい差異がみられ，Wagener等が，5年間死亡率Ⅰ群30％，Ⅱ群46％としたのに対して，それぞれ15％．27％（Frant等），10％．25％（Palmer等）などの報告がある。

Authors investigated prognosis of life in the patients of benign hypertension with retinal changes of Fundus hypertonicus (Thiel, Amsler). Using "return posrcards," 170 cases were inquired into the latest state of health at the time five years after first examination of fundus and following informations were given from 101 cases.

1. Death-rate of the patient with Fundus hypertonicus within five years was, on the whole, 10.8 per cent. According to Keith-Wagener's criterion, mortality-rate were 2.4 per cent in cases of Group I and 16. 6 per cent in cases belonging to Group II.
