脊髄腔内空気注入が奏効したと思われる球後視束炎の1例—多発性硬化症との関係

Retrobulbar Neuritis with Good Effect of Air Infusion into the Spinal Cavity

　球後視束炎の病因論に就いては，桑島の主唱以来多発性硬化症が^1）2），従来最も重要視されて来たビタミンB₁欠乏症に大きな揺ぎを与えているが，一方本疾病の原因的要素が多様性を有していることも現在否定し難い所である^3）。

　本症例は両眼性の定型的球後視束炎と診断されたもので，一側の腹壁反射を欠き，一側の膝蓋腱反射の亢進を示したものであるが，背髄腔内空気注入を契機として，急速な視力回復を見た。「多くの神経障碍症状がしばしば機能的回復を示すことは，多発性硬化症の特長であるから，殊殊治療の効果判定に頼つてその原因を推測することは，多発性硬化症を除外する根担になり得ない」（桑島）^1）と云う事を充分考慮しつつ，本症例の経過並びに原因を述べて見たい。

Female aged 16. Her visual acuity had greatly fallen since she was attacked with fever about 2 months before. Her visual acuity was 0.07 D in both eyes. A racket-like central scotoma was observed. The fundus was normal. Diagnosis was bilateral retrobulbar neu-ritis. There was suspicion of multiple sclerosis since reflex of the left abdominal wall disappeared and the left patellar reflex remarkably increased as general nervous symptom. Such first trials as lumbul punction and pumping produced no sign of recovery of visual acuity.
