（45）星状神経節遮断の網膜血管径に及ぼす影響

Stellate-ganglion Block's Effects on Calliber of the Retinal Blood Vessel

　星状神経節のノボカイン遮断術は，脳血行障碍に基く頭蓋内病的状態の積局的療法として，欧米に於ては一般に広く臨床に応用され，その効果はほゞ確認されるところで，近時我国に於ても脳神経科方面に時々報告が見られる。

　眼科領域に於ても種々の疾患に用いられ，その効果が論ぜられている。Scott （1951）は脳血栓に由来すると思われる同名半盲に応用して劇的効果の見られた例を報告しているし，Coston （1951）は網膜動脈エンボリーで視力の改善と網膜動静脈に於ける著明の拡大，塞栓の末梢えの移動とを認めた例を記載している。然しKennedy，McGannon （1952）等は，網膜色素変性症の14例に本法を施行して何等の客観的改善はなく，自覚的改善はあつても視力，視野に改善の見られたもののないことから，純心理的影響に過ぎず，本症治療として本法は無価値と信ずると述べている。吾国に於ても河本郁雄（昭28）は球後視神経炎，調節不全麻痺，良性高血圧性網膜出血，色素性綱膜炎及びバセドー氏病に於ける眼球突出等に有効の事を，尚，八木睦夫（昭28）は網膜色素変性症に本法と0.1％Procain生理的食塩水溶液1000c．c．の静脈注射併用により，視力に飛躍的改善を見られたものを記載する等多少の報告がある。

The stellate-ganglion block of 1% novocain solution causes expansion of calliber of the retinal blood vessel together with Horner's syndrome of miosis, ptosis etc., hyperemia of the conjun-ctiva, or burning hot of the face.

1) Studies of six cases of surgical patients with normal eyes have provided the following result:

The retinal blood vessel began expansion after 5~10 minutes (average 7.5 minutes). Expan-sions of artery (4.7~15.4%, average 8.5%), vein (0~7.5% average 4.3%) continued. 20~25 minutes (average 22 minutes) and 0~30 minutes (average 16 minutes) respectively.

2) Pigmentary degeneration of the retina (2 cases)
