（46）眼瞼に観られた前癌状態？に就て—第1報 マイボーム腺の態度

Precaficerous Condition of Eyelid (Report on Condition of Meibom's Glande)

　眼部癌の内，眼瞼に発生する癌は眼瞼皮膚とか瞼板の様なマイボーム腺（以後マ腺と略称）特にマ腺の如き或は其他の皮膚腺の如き腺細胞集団の多い場所に発生するものが臨牀上比較的多い。従つて或るものは麦粒腫の如く，或るものは霰粒腫の如く発育して来るものがあつて注意深く観察すればよいが時には切開によつて再発，増殖を来す傾向を示し初めて癌腫であつたと云う事が知られる場合がある。一方之に反して眼瞼結膜に発生する癌症例の報告は極めて少く，吾が眼科領域では現在まで僅に7例報告されたに過ぎない。従つて今迄の多数の報告例は何れも眼瞼癌と称するもので殆んどすべてが腫瘍としての形を完備している。最近私共は瞼結膜面に発生した極めて浅い潰瘍から興味ある所見を得てその経過を観察中眼瞼縁に極めて小さいポリープ様の隆起の発生を見たので，之等病的の部分を瞼板の一部と共に健康部も附けて切除した。これが組織学的検索において癌化の初期と思われる種々な興味ある所見をマ腺に於て観察し得たのでこゝに報告して諸賢の御批判を仰ぐ次第である。

Patient : Women, 95years old.

We have examined a test tissue of ulcus of slight nature which developped on the left upper conjunctiva palplebrae and discovered mingling of epithelial cells with a very large kernel.

After the elapse five weeks, around the opening of Meibom's gland where the ulcus had developped we found three to four polp-like small protuberances. Although no changh took place to Meibom's gland, a part of epithelium showed the trends of degeneration and necrosis, besides beneath the epithelium in-filtration of small round cells was recognized.
