緑内障眼前房隅角の微細構造—（5）アミロイド緑内障

The Fine Structure of the Iridocorneal Angle Tissue in Glaucomatous Eyes:(5) Glaucoma Secondary to Primary Familial Amyloidosis

緒言

　原発性家族性アミロイドージスに開放隅角緑内障が続発することはKaufmanの報告^1）以来よく知られているが，本疾患が地域特異性が高くまれな疾患であるためこの型の緑内障に関する報告はきわめて少ない^2）。

　著者は今日までにこの稀有なる緑内障症例を5例経験しその前房隅角組織を電顕にて観察する機会を得た^3〜5）。その結果原発性家族性アミロイドージスに続発する開放隅角緑内障はアミロイド緑内障とも呼ぶべき特異的な前房隅角組織の微細構造変化を呈することを確認すると同時に，開放隅角緑内障の流出抵抗増大の場は内皮網であることを強く示唆する所見が得られたのでここに報告する。

Recently, the author experienced 5 cases of open angle glaucoma secondary to primary familaial amyloidosis and performed trabeculec-tomy to 2 cases of the disease. The trabeculec-tomy specimens obtained were examined with a transmission electron microscope in order to clarify the pathogenesis of this type of glau-coma.

Significant deposition of amyloid material was found in the endothelial meshwork of the trabecular tissue in both cases. This finding strongly suggests that the material which plays an important role in outflow facility reduction in this type of glaucoma is the amyloid material in the endothelial meshwork.
