緑内障眼前房隅角の微細構造—（4）原発性開放隅角緑内障

The Fine Structure of the Iridocorneal Angle Tissue in Glaucomatous Eyes.:(4) Primary Open Angle Glaucoma

緒言

　開放隅角緑内障眼の内皮網内に線維要素の著明な増加沈着が観察されているが^1）2），それが眼圧上昇の原因であるのか，それとも結果であるのか，あるいは加齢変化であるのかという問題は現在のところ未解決である。一方前報^3）にてSch—lemm管線維柱切除術（Trabeculectomy）のさい得られる切除組織片がほぼ完全な形でSchle—mm管ならびに線維柱網状組織を含有しており，前房隅角組織の電顕的研究にも十分利用できることが明らかにされたので，今回はTrabeculecto—my組織片を透過電顕にて観察し，原発性開放隅角緑内障眼の内皮網内変化を解明しようと試みた。観察された緑内障眼は23眼（年齢20〜76歳，視野湖崎氏分類II-a—VI期，C値0.02〜0.20）で，この観察から原発性開放隅角緑内障眼の内皮網にみられる変化のうち，Schlemm管内壁直下への無定形物質の増加沈着は，年齢，病期よりはむしろ房水流出率と相関関係があることが判明したのでここに報告する。

Trabeculectomy specimens obtained from 23 primary open angle glaucomatous eyes were examined with a transmission electron microsco-pe and morphomery of the endothelial mesh-work was done in 11 eyes. Before operation, outflow facility had been measured in all eyes. All glaucomatous eyes showed deposits beneath the trabecular wall endothelium of Schlemm's canal. The amount of subendothelial deposits was reversely correlated with outflow facility.
