緑内障眼前房隅角の微細構造—（3） Schlemm管線維柱切除術

Trabeculectomy Specimens : A Transmission and Scanning Electron Microscopic Study

緒言

　原発性開放隅角緑内障眼の前房隅角組織は電顕レベルでも研究が行なわれつつある。しかしながら，通常これらの電顕試料は死後摘出眼から得られるため，観察された病変が死後変化そのものであるとか，経年変化であるといつた反論が起こつてくる。電顕的研究のためには手術摘出眼より試料を作ることが理想であるが，緑内障眼の場合それはほとんど不可能なことである。唯一の手段は，緑内障手術時に得られる生検試料が電顕的研究に利用できないかということである。過去において管錐術のさい得られる組織片を電顕的研究に利用しようと試みたことがあつたが，その内部に前房隅角組織を認めることはできなかつた。Zim—mermanも管錐術試料が光顕的研究にすら不適であると述べている^1）。しかしながら近年緑内障手術にも新しい手術法が導入され好成績をあげつつある。Schlemm管線維柱切除術（Trabec—ulectomy）もその一つである。今回はTra—beculectomyによつて得られた組織片が電顕的研究に利用可能かどうかという問題を，従来の手術摘出眼より得られた試料と比較しつつ検討し，この手術によつて得られる試料が電顕的研究に十分利用可能であることを確認した。

Trabeculectomy specimens obtained from glaucomatous eyes (4 primary open angle glau-comas and 2 unilateral open angle glaucomas) were examined with both transmission and scanning electron microscopes and compared with specimens taken from surgically enucleated eyes.

In trabeculectomy specimens, mechanical dam-ages caused by surgical procedures and mor-phologic changes which seemed to occur as a result of an inversion of the normal pressure gradient were found. The damages were so minimal that they were not serious for electron microscopy.
