緑内障と虹彩萎縮を主徴とする特異な急性虹彩毛様体炎

Acute Iridocyclitis, a Peculiar Form Accompanied by Acute Glaucoma and Iris Atrophy. A Report of the Cases

緒言

　激しい急性虹彩毛様体炎に急性の眼圧上昇を伴い，あとに斑状の脱色素性虹彩萎縮を残した特異な虹彩毛様体炎3例を最近経験した。この症例は，その経過中に角膜後面に無数の色素顆粒の付着がみられ，さらに隅角検査によつて隅角線維柱帯上に多量の色素沈着がみられた。このような所見から，本症例は虹彩毛様体から多量の色素脱出をきたしたのが主病変であり，脱出した色素が角膜後面に沈着してプレチピタートとなり，また，隅角線維柱帯上に沈着して房水流出をさまたげて高眼圧をもたらし，さらにあとに虹彩に脱色素斑を残したものと思われる。

　色素性緑内障にみるごとく，前房隅角における色素沈着は，緑内障発生機序の一つとして従来から注目されているが，ここに示す症例もそのいみで興味深い。

Three cases of acute iridocyclitis which had common peculiar clinical features were observed in our clinic. While the clinical pictures of the cases resembled those seen in herpes zoster ophthalmicus, the cases showed neither skin eruption, nor outer eye involvement.

Characteristics of the cases were followings. Severe iridocyclitis occurred abruptly in unila-teral eye of otherwise healthy persons in mid-dle age ranging from 32 to 58 years. All of them complained heavy migrane at the time of initial stage.
