高眼圧と虹彩萎縮を伴う片眼性の急性虹彩毛様体炎は，ウイルス感染によるものと考えられている．今回，我々も同様の症例を経験し，手術時に得られた虹彩および隅角部の組織学的検査を行い，その病態について検討した．
電顕により虹彩には肥大化したメラニン細胞が観察され，血管内皮細胞，上皮細胞内に直径150nmの三層構造を有する均一な顆粒が多数認められた．これは形態的にヘルペスウイルス様粒子と考えられた．さらに螢光抗体法で虹彩には単純ヘルペスウイルス抗原の存在が認められた．このことは本症例が単純ヘルペスウイルスの感染による虹彩毛様体炎である可能性を強く示唆するものである．
A 42-year-old female presented with mild iridocyclitis with elevated intraocular pressure in her right eye of 2 weeks' duration. Iris depigmenta-tion developed during the ensuing 10 weeks. We performed trabeculectomy to relieve persistent ocular hypertension in spite of intense treatments with topical corticosteroid and systemic acet-azolamide.
Electron microscopy of the resected iris showed numerous herpes virus particles in the capillaryendothelium, melanocytes and the pigment epithe-lium. No virus particles were detected in the ante-rior chamber angle. Indirect immunofluorescent antibody method using anti-herpes simplex anti-sera showed positive reaction tissues similar to the electron microscopic findings.
The present case illustrates that infection by herpes simplex virus may cause iridocyclitis as-sociated with ocular hypertension and depigmenta-tion of the iris.
