小児におけるステロイド白内障の臨床的検討

Posterior Subcapsular Cataracts in Children on Long-Term Corticosteroid Therapy

緒言

　今から約20年前，Henchらが，リウマチ様関節炎の炎症症状にコーチゾンをはじめて用い，著明な効果を得て以来，副腎皮質ホルモン剤が臨床的に使用されるようになつた。わが国においても約15年前よりその使用が普及し，現在は多くの薬剤のうちでも最も重要なものの1つにかぞえられているが，その反面，副腎皮質ホルモンによる副作用が増加してきており，大きな問題となつている。このうち，眼科領域においては，ステロイド白内障，ステロイド緑内障，ステロイド離脱時のReboundなどが問題となつている。なかでもステロイド白内障は，その診断基準や年齢関与，病状進行性の有無，手術的治療法の適否など，いまだ未解決の点が多く残されている。このような背景のもとに，最近われおれは，副腎皮質ホルモンを使用している109例と，副腎皮質ホルモンを使用したことのない対照の71例の小児を対象として眼科的検査を行ない，ステロイド白内障の発生について若干の知見を得たので報告する。

A total of 109 children who had been on long-term corticosteroid therapy were examined for posterior subcapsular cataract (PSC). They comprised 81 patients with nephrotic syndrome, 7 with chronic nephritis, 6 with bronchial as-thma, 5 with purpura nephritis, 4 with rheu-matoid arthritis, 4 with systemic lupus erythe-matosus, and 2 with congenital adrenal hyper-plasia.

The results were as follows.

1. PSC was detected in 24 cases (22.0%). The general diagnosis was nephrotic syndrome in 18, chronic nephritis in 2, purpura nephritis in 1, rheumatoid arthritis in 2, systemic lupus erythematosus in 1.
