進行性錐体ヂストロフィー

Progressive Cone Dystrophy

　網膜広範囲の錐体機能が強く障害された4症例を記述した。発症年齢は後天性に，10歳台ないし30歳台であり，症状は緩慢に進行した。主症状は視力低下，色覚異常，昼盲であり，夜盲はなく眼振もなかつた。暗順応，ERG検査などにより広範囲の錐体機能の異常と，正常な桿体機能が認められた。4症例とも単発例であり，2症例の両親には血族結婚が認められ，常染色体性劣性遺伝による網膜のヂストロフィー的変化が考えられた。

　これらの症例は，欧米において報告されている進行性錐体ヂストロフィーに類似していたが，眼底にほとんど異常が見出されないことや，EOGが正常のことなどから，それらの病型の異型か，または新しい疾患である可能性が考えられた。

Four cases were reported having selective involvement of cone mechanism. Their ages at the initial visit were 23, 22, 45 and 41 yearsrespectively, two males and two females. Visual disturbances began at second to fourth decade of life, after some period of normal vision, and the main symptoms were poor visual acuity, acquired color vision defect and daylight blindness, whereas scotopic vision remained intact. Dark adaptation and ERG studies revealed severe involvement of the cone mechanism throughout the entire retina with little or no defect of the rod mechanism.
