アルゴンレーザー光凝固について—その1．試作光凝固機における諸問題

Argon Laser Photocoagulation:Part 1. Design and Evaluation of Slit-lamp Photocoagulator

緒言

　アルゴンレーザー光凝固については，すでに多くの報告^1）があり，とくに1970年にLittle^2）等によつて細隙灯顕微鏡とアルゴンレーザーを組み合せた実用型の光凝固機が製作されてから急速に発達した。最近では，主としてアメリカにおいてアルゴンレーザー光の特性，すなわち凝固能率の良いこと，高エネルギーで集束性の強いこと，血液に吸収率の高いこと等を利用して後極部の精密凝固および小血管の凝固閉塞に応用されている^3）。またわが国でも昨年からCoherent Ra—dition Model 800アルゴンレーザーコアグレーターが試用され好結果をえている。

　著者は，すでに1968年に国産のアルゴンレーザー装置を用いた光凝固の実験について報告したが，えられた出力が少なく（最大100 m．W．以下）実用にはいたらなかつた。しかし最近装置の主要部である放電管の国産化が完成し，アメリカと同様に2Wのアルゴンレーザー装置が比較的容易に製作されるようになつた。私たちは，これらの装置を利用してアルゴンレーザー光凝固機を試作し実験を行なつた。

The argon laser photocoagulator which can be used in clinical application was designed and constructed. The instrument utilized slit-lamp as an observation and coagulation optics, which is connected to the laser by optical anc-le. In clinical trials, the author confirmed that the argon laser coagulator has capability to pro-duce small coagulation size (below 0.5°), wh-ich is more feasible to coagulate macular area.
