糖尿病性網膜症の分類と病態—新しい分類の提案と文献的考察

緒言

　1875年にLeberが糖尿病に合併する網膜病変をRetinitis （Retinopathia） diabeticaと命名し，さらに1877年にMachensieが糖尿病患者に毛細血管瘤を認めて以来，糖尿病に合併する網膜病変が注目されるようになり，1890年にはHir—shbergによつてその全容がほぼ明らかにされたが，その後糖尿病性網膜症には異なつた2種類の病変が存在することがわかり，1953年のEhlers以来，糖尿病性網膜症を単純型病変と増殖型病変に分けて考える人が次第に多くなつてきた。一方網膜病変を進行状態に応じて把握しようとする試みも行なわれ，1934年のWagenerの分類をはじめとして多くの人によつて病期分類が行なわれてきた。

　病期別分類は第1表に示したごとくに「個々の病変の進行程度に重点をおくか」あるいは「網膜症全体の進行程度に重点をおくか」によつて2つに大別される。さらに後者は網膜症を完全に増殖型と単純型に区別しているか，あるいは不完全に区別しているか，または全く区別していないかによつて3つに細分されるが，これは「増殖型病変が必ず単純型病変の存在下に発生する」という所見に対する解釈の仕方の相違によるものである。

A clear distinction has to be made between simple and proliferative diabetic retinopathies. The simple diabetic retinopathy is characterized by its benign clinical course so that the vision is well retained over several years. It is regard-ed as an instance of retinal capillaropathy. The proliferative diabetic retinopathy is charac-terized, on the other hand, by its progressive tendency and for the poor prognosis as to vi-sion. It may occur spontaneously and is not ne-cessarily the consequence of vitreous hemor-rhage.
