Diabetic Retinopathy and O'Hare Classification

緒言

　Background retinopathy^1）は，血管新生，fi—brous proliferationをおこす以前の網膜症を総括して扱つている。増殖性網膜症は，血管新生のlife cycleに相当するものであり，Malignantretinopathyとして扱われる。従来，増殖性網膜症（stage V）の前期（stage IV）として硝子体出血をこれにあてているが，これは血管新生＋BeadsといったKeith-Wagener分類等がこの意味で合理的といえる。光凝固法の利用から特に，その悪性化の防止にも血管新生のlife cycleとしてその初点が目標となつている。したがつて，増殖性網膜症もearly surface Neovascula—rization, moderately advancedまたはseve—re phase, late phaseと細分していく必要もでてくる。また，Meyer-Schwikerath^2）のように光凝固や，BeethamのようにLaserをBackgr—ound retinopathyに応用するようになると，眼底パターンのもつ意味を究明する上にも，一定の標識化が要請されてくる。

The classification of diabetic retinopathy (O' Hare) is discussed with regard to the grading by Scott and to the course of evolution of re-tinopathy. It is proposed that circinate retino-pathy, which was referred en bloc to background retinopathy by O'Hare, be treated as a separate item. Circinate phase is thus to be designated as C₁, circinate lipid aneurysm complex as C₂ and large plaque as C₃. From the therapeutic point of view, by means of photocoagulation in particular, the classification by O'Hare is rega-rded as more valuable because of its more precise description.
