緒言

　球後視神経炎は原因の不明なものが多く，ステロイド等の投与により，原因の判らぬまま症状の好転する例を少なからず経験する。われわれは最近，球後視神経炎の診断のもとに，ステロイドの結膜下注射，内服等で1年以上ものあいだ寛解と再発をくりかえし，視野および頭部X線像，脳血管像等より下垂体腫瘍を疑つて開頭した結果，後部副鼻腔の術後性嚢腫の圧迫による，いわゆる球後視神経炎と判明した興味ある症例を経験したので報告する。

A report is presented on a 31-year-old male with bilateral retrobulbar optic neuritis caused by postoperative mucocele of the posterior ac-cessory nasal sinuses. While systemic corticos-teroid was initially effective, serious visual im-pairment developed after temporary withdrawal of the corticosteroid. The findings were strong-ly suggestive of pituitary tumor because of the presence of bilateral atypical hemianopsia, des-tructive enlargement of the sella turcica and oculomotor paralysis that developed later.
