We evaluated the surgical outcome in 14 cases of optic neuropathy secondary to paranasal lesions. Extirpation of paranasal cyst was performed as the standard procedure. Unilateral involvement was the rule in the present series. While the visual acuity improved after surgery in 13 eyes, the out-come was less favorable than that in other series of idiopathic optic neuropathy. The final result was less favorable in eyes with eroded optic canal. The interval between onset and surgery did not affect the visual outcome. Besides direct compression by the paranasal cyst, inflammatory and circulatory disorders seemed to have contributed to the path-ogenesis of optic neuropathy.
