　当眼科を受診し，副鼻腔疾患を認め，耳鼻科で副鼻腔嚢胞の診断で手術を施行した視神経障害患者14例14眼について検討した。視力は1眼を除いて全例2段階以上改善したが原因不明の視神経炎より不良であった。視野は術後12眼が改善し，悪化したものはなかった。視神経乳頭は術前から全蒼白のものは視力予後は不良であった。画像診断では，視神経管破壊は7眼で確認され，手術時では8眼に確認された。視神経管骨欠損のある例は視力予後が悪かった。発症から手術までの期間は，早期に手術したにもかかわらず予後の悪い例もみられ，嚢腫による圧迫だけでなく，炎症・循環障害等の影響も考えられた。

We evaluated the surgical outcome in 14 cases of optic neuropathy secondary to paranasal lesions. Extirpation of paranasal cyst was performed as the standard procedure. Unilateral involvement was the rule in the present series. While the visual acuity improved after surgery in 13 eyes, the out-come was less favorable than that in other series of idiopathic optic neuropathy. The final result was less favorable in eyes with eroded optic canal. The interval between onset and surgery did not affect the visual outcome. Besides direct compression by the paranasal cyst, inflammatory and circulatory disorders seemed to have contributed to the path-ogenesis of optic neuropathy.
