螢光眼底撮影法—その6視神経乳頭の螢光像

Fluorescein Fundus Photography. Part 6. Fluorescence of the Optic Disc

緒言

　螢光眼底造影法が網膜のみならず，視神経乳頭の微小循環の解析や，その病変の補助診断法として利用できることは，すでにいくつもの報告があるが，その応用は主としてうつ血乳頭などに限られていた。ことに，視神経乳頭の螢光像を，乳頭内の血管構築や，乳頭周囲の脈絡膜血行と関連させて解釈しようとする試みは，近年，緑内障眼などではじまつてはいるものの^2）4），まだ緒についたばかりであり，特に，正常眼での螢光所見と関係してとられていないため，まだ多くの問題点を残している。

　著者は新しいフィルターの組み合わせと，頻回螢光撮影手技とを採用することにより，従来の方法では得られなかった視神経乳頭の螢光を記録解析することに成功し，とくに，正常眼についても，乳頭そのものの螢光に，その血管構築と関連したいくつかの型があること，そして，乳頭内血行は，脈絡膜循環と密接な関係のあることを発見した。これらの知見は，今後，各種の病的状態における乳頭の螢光像を解釈する場合にも，つねに留意されるべき基準であるとみなされる。

Fluorographic findings of the optic disc are discussed as studied by means of rapid serial microangiographic techniques. Introduction of highly selective interference filters has been essential in eliminating pseudofluorescence of the disc.

Four different types of disc fluorescence could be differentiated according to the vessels of origin. (1) Deep fluorescence of the disc appe-ars as vague, ill-defined one. Its appearance is related with that of choroidal flush and is fre-quently earlier than the dye inflow into the central retinal artery.
