視神経乳頭の螢光漏出像の検討

　原田病，ベーチェット病，その他のブドウ膜炎，および視神経炎の炎症性疾患と，網膜中心静脈閉塞症，網膜静脈分枝閉塞症，高安病，うつ血乳頭の各疾患に螢光眼底撮影を施行し，乳頭の螢光漏出像の型の比較検討を行なつた。

　ベーチェット病，その他のブドウ膜炎の中で虹彩毛様体炎が主体のもの，視神経炎，網膜静脈分枝閉塞症，高安病では，ほとんどの症例で，乳頭の中心部にある乳頭血管から漏出する中心型を呈した。それに対して，原田病およびその他のブドウ膜炎の中で眼底変化の強いものでは大多数が周辺型を呈した。うつ血乳頭，網膜中心静脈血栓症では，経過観察が不十分なものが多く，乳頭表層の血管から強い螢光漏出を生じるものが多数であつたが，その他の症例を比較すると，前者が周辺型ないしは周辺型類以の漏出を示したのに対して，後者には中心型が多く見られた。

　疾患によつて，乳頭の螢光漏出の機序に差異があることが明らかになつた。

Fluorescein angiography frequently shows the papillary leakage in various diseases. Therefore;in order to elucidate the difference of the papil-lary fluorescence pattern, fluorescein angiograms were studied in 59 cases of Behçet's disease, 34 cases of Harada's disease, 46 cases of other uveitis entities, 20 cases of neuritis, 22 cases of choked disc, 20 cases of central retinal vein occlusion 53 cases of retinal branch vein occlusion and 9 cases of Takayasu's disease.

The fluorescence pattern of the papillary leakage was classified into the following four types : 1. peripheral type. 2. central type. 3. segmental type. 4. unknown. In peripheral type, the fluorescence appears from the papillary margin and forms a complete ring in the shape of halo. In central type, the fluorescein dye leaks out from the vessels of the central part of the disc, while the dye appears from the vessels of the disc near the papillary margin in segmental type.
