網膜剥離手術と渦静脈

Surgery for Retinal Detachment and Vortex Vein

緒言

　網膜剥離の場合，網膜裂孔は一般に網膜周辺部に見出されることが多い。しかし時として裂孔の位置が渦静脈の強膜貫通部位近くの網膜に認められることもあり，このような網膜剥離は手術の際渦静脈が妨げになり，著しく複雑困難になる。すなわら，裂孔がかなり深い位置（網膜中心部寄り）に存在するためにdiathermyを行なう際には，強膜をかなり深部まで露出せねばならず，しかも渦静脈に手術侵襲を加えぬように注意を払いながら，的確に裂孔閉塞を行なわねば手術の目的は達せられない。また強膜に対する種々の補助千術（scleral infolding,scleral buckling,equato—rial ring operation等）の場合も，渦静脈損傷の危険が大きいが，これは絶対に避けねばならぬ。またlight coagulation,cryoapplicationの場合も渦静脈は，常に損傷の危険にさらされており，手術法の如何にかかわらず慎重な手術を要求されることはいうまでもない。

　著者は過去において渦静脈近くにある，網膜裂孔により発生した剥離に対して手術を行ない，渦静脈に損傷を加えるという苦い経験を味わったことがある。以下に渦静脈損傷2例を紹介し，あわせて最近著者が工夫採用している渦静脈損傷防止策について述べる。

Surgery for retinal detachment is difficult, when retinal break is situated near the vortex vein. The author reported about injury of vor-tex vein in retinal detachment surgery and de-vised some methods to prevent vortex vein's injury.

The author operated two cases of retinal de-tachments with retinal breaks near the vortex veins and gave excessive elongation and comp-ression for vortex veins. As a result of such injuries, chorio-retinal circulation was strongly disturbed. So, one case ended in blind but ano-ther one recovered luckily by post-operative treatment.
