Significance of Clinical Tonography on the Diagnosis of Glaucoma

緒言

　緑内障診断において眼圧調整機能障害を認知する上で，臨床トノグラフィーは重要な役割を果している。Moses, R．　A．＆Bruno, M．（1950）およびGrant, M．（1950）がトノグラフィーの概念を導入し，Grant原法が臨床に応用されてから数多くの研究者によってその信頼度，問題点が列挙され，また改良を目差す種々の方法が提起された。換算表に関しては1955年眼圧NomogramがFriedenwald, J．　S．によって確立されたものを現在では国際的に採用し統一されているが，実施方法や解析法については今なお評価が定まっていないと思われる。

　従って本論文においては先ず正常眼と緑内障眼の多数例について，Grant原法と比較してトノグラフィー施行時間，rigidity補正，圧補正の点を詳細に解析し，緑内障診断の上での意義を評価することを試みた。

1. Various methods of clinical tonography have been analyzed as to their value in diffe-rentiating glaucomatous eyes from non-glauco-matous ones. Representative cases from a seri-es of 3,500 tonograms served as basis for the present study. Following calculations were made on each tonogram:

C_0-4:classical tonography for the initial 4 minutes.

C_0-5:same as above for the initial 5 mi-nutes.

C_0-4C: same as above corrected for scleral rigidity.

C_0-5C:same as above for the initial 5 mi-nutes.

C_0-5M:C_0-5×18/Po (after Merté, 1965)

C_3-7:"tonography test" after Leydhe-cker.
