縮瞳剤治療中における散瞳トノグラフィー

Mydriasis Tonography in Miotic Regimen

I．緒言

　緑内障ですでに縮瞳剤治療を受けている患者を初診に遭遇することがしばしばある。そのさい治療前の眼圧や房水流出状態を知り得ないことが多い。

　開隅角眼における縮瞳剤の眼圧下降作用機序はまだ解明しつくされていないが，大部分の症例に著明な房水流出の改善をみることが1950年Grantのトノグラフィーによる観察以来知られている。他方また，毛様弛緩作用の散瞳剤が隅角を機械的に閉塞することなしに房水流出抵抗を上昇させ得ることが知られている（Galin 1961,Christensenら1963,Bárányら1967，真壁1966,1968,1969，岩田ら1968など）。

In 35 eyes with open angle glaucoma, which had regularly been treated with topical miotics (pilocarpine, prostigminc or Glaucostat), a de-crease of the facility of outflow with a rise ofIOP could be elicited following the instillation of 1 % Cyclogyl, 1 homatropine or 0.5 tro-picamide which are parasympathicolytic agents. This phenomenon could not be observed follow-ing the use of 10% Neo-Synephrine which is a sympathicomimetic mydriatic agent.
