典型的な進行性糖尿病性網膜症（Scott Ib型）の1例

Clinical Studies in A Case of Proliferative Diabetic Retinopathy

I．はじめに

　糖尿病性網膜症（以下網膜症と略）のなかには，比較的太い網膜血管系の異常—特に広範な血管新生—を主要病変とした進行性の強い一群があること^8）11）が従来知られていた。Scott^{16）〜18）}はこれをIbからVbに至る特別な1系列の病型と考えて，ほかの一般的な経過をとるものと区別することを提唱したが，この型のものは短時日のうちに失明に至る危険率のきわめて高いため，その治療的対策には，下垂体手術^{2）7）10）23）}や網膜光凝固療法^12）15）などの多くの苦慮が払われてきた。

　最近われわれはこのScott Ib型^17）と判定し得る典型的な進行性網膜症を有する若年型糖尿病の1症例を経験し，全身療法の改善強化と網膜光凝固療法とにより，その進行を一応阻止し得たので，ここに報告したい。

A report is presented over a case of prolifera-tive diabetic retinopathy (Type lb after the classification of Scott) as studied for more than a year during its progressive stage.

The patient, a female of 26 years of age, was diagnosed as juvenile diabetes mellitus with diabetic retinopathy in its early stage. During the following three months of clinical observation, multiple neovascularisation burst forth in the peripapillary retina in both eyes. In the left eye, preretinal hemorrhage appeared in the posterior fundus.
