糖尿病者の眼圧

The Ocular Tension in Diabetes Mellitus

I．緒言

　糖尿病患者の眼圧の問題はHeine （1903）^1），Krause（1904）^2）が糖尿病性昏睡時に著しい低眼圧を示す症例を報告したのに始まるが，その後あまり注目されなかつた。1920年代に入つて，血糖と眼圧の変動の関連が報告された。糖尿病者では眼圧の変動が一般に大きいといわれ，また糖尿病性網膜症の有無によつて差があるともいわれている．このような糖尿病者における眼圧の変動やくり返す低眼圧が網膜症発症進展のfactorになりうるという報告も多い。糖尿病と緑内障との合併は，出血性や続発性緑内障については古くより知られているところであるが，原発性緑内障については，Armstrong （1960）が糖尿病者では単性緑内障の合併率が一般対照より高率であると報告して以来，それを支持する報告は多い。

　しかし実際臨床上，糖尿病者の白内障や白内障手術例にはよく遭遇しても，原発性緑内障の合併，特にその手術例はほとんど経験しない。糖尿病者の眼圧には多くの要因の介在が考えられるが，現在なお相反する意見が多く，不明の点が多い。

The intraocular pressure and the incidence of glaucoma was studied in a total of 417 diabetic subjects (816 eyes) in which 248 eyes (31%) showed various degrees of diabetic retinopathy. Overt cases of hemorrhagic glaucoma, eyes sub-jected to surgery and refractive anomalies of more than 6 diopters were excluded from the present study.

1. The average IOP was higher in cases without retinopahty (males : 151mmHg, fe-males : 15.5mmHg) than those with retinopathy (males : 14.4mmHg, females : 14.9mmHg).

2. The IOP tended to decrease with age and Particularly in the group over 60 years.
