外転神経麻痺を主症状とした上咽頭悪性腫瘍の3例

Three cases of the Epipharyngeal Tumor with Abducensparesis

I．緒言

　上咽頭腫瘍がしばしば脳神経症状（その多くは眼症状であるが）を伴うことは1922年G．B．Newの報告以来幾多の報告がみられるが，本疾患は本来耳鼻科領域のものであり，かつ比較的少ない疾患であるため耳鼻科以外の脳外科，眼科に属するわれわれが直接これに接することはまれである。一方外転神経麻痺は眼科はもちろんのこと，脳外科領域においても，しばしば遭遇する症状ではあるが，外転神経麻痺からただちに上咽頭腫瘍ではなかろうかと予想することは一般にはまずない。われわれは最近外転神経麻痺を主症状とし，耳・鼻症状はないかあるいはきわめて軽微であった上咽頭悪性腫瘍を3例経験した。いずれもレ線学的に診断し，ついで耳鼻科での試験切片について組織診断を行ない，Co⁶⁰の放射線治療を施行した。3症例について症状，経過ならびにレ線学的診断などを中心に報告し大方の参考に資したい。

Three cases with abducens paresis due to epi-pharyngeal tumor are presented. Two were males of 51 and 41 years each and the other was a female of 33 years of age. The initial complaints consisted of diplopia, headache, tin-nitus and nasal discharge. Neurological exami-nations further revealed abducens paresis and hypesthesia in the ipsilateral cheek. The x-ray planographic studies revealed destructive pro-cesses of the body of the sphenoid, the posterior clinoids or the sella turcica together with irreg-ular contour of the epipharynx. A mass-like shadow was present near the sphenoid in the 2nd case.
