直視型圧迫隅角レンズの試作

An Attempt of Contact Lens for Direct Indentation Gonioscopy

I．緒言

　狭隅角に対して，隅角を開大して観察しようとする試みは，従来，諸家によりいくつかの方法が考えられ，なされてきた。その中で，角膜に圧迫を加えることにより房水の移動をはかり，隅角へ移動した房水によつて虹彩根部を押し下げて，その後方にかくされた癒着などの所見を観察するという考えは特にすぐれている。私は直視型隅角レンズにこの原理を応用し，実用に耐え得るレンズを試作したので，ここに報告する。なお，この原理を応用した隅角検査法は，岩田氏が，「圧迫ゴニオスコピー」と命名しているので，これに従うこととする^2）。

A direct-image indentation goniolens was de-veloped by the author. The apparatus is a modified Koeppe goniolens with smaller dia-meter of the concave surface facing the cornea and a small lever so that selected areas of the cornea can be indented to cause a widening of the chamber angle in the opposite. Examination of the chamber angle is then conducted with the use of a lupe (Keeler) or with an operation microscope. Preliminary use of the new goniolens in a case of acute angle-closure glaucoma proved promising in the easy and exact differentiation of functionally vs.
