合併散瞳暗室緑内障負荷試験—その2緑内障眼における成績

Combined Mydriasis-Darkroom Test.:Renort 2 : Findings in Glaucomatous Eyes

I．緒言

　前篇に合併点眼散瞳暗室試験を考案し，十分多数の正常眼に試みて，生理的眼圧上昇限界値を推計学的に算出した。作用時間が短く，効果も比較的軽度の副交感まひ剤Mydriaticum-Rocheであるが，その点眼と暗室試験の併用で，正常眼に平均約3mmHgの有意の眼圧上昇を認め，圧上昇7mmHgまたはそれ以上を疑い，圧上昇9mmHgまたはそれ以上を確実に病的と見なし得ることが知られた。

　本篇では初期緑内障眼（広隅角単性緑内障および狭隅角緑内障）における成績を報告し，特に広隅角単性慢性緑内障について検討する。

Eighty-two eyes with glaucoma were testedwith provocation test consisting of medical wi-dening of the pupil with Mydriaticum-Roche followed by confinement in darkness for one hour.

1. In 40 eyes with incipient simple glaucoma with wide chamber angle, the initial intraocular pressure level lay between 16 and 26mmHg and averaged 21. 4mmHg. The induced rise in IOP was 7mmHg or more (m+2s) in 12 eyes and 9mmHg or more (m+3s) in 9 eyes. The fiducial limits of the frequency of pathological reaction are at the significance level of 5% for the 2s-limit 17.4~45.5% and for the 3s-limit 11.5~37.3%.
