合併散瞳暗室緑内障負荷試験—その1正常眼における成績

Combined Mydriasis and Dark-Room Test.:Report 1:Findings in Normal Eyes

I，緒言

　点眼散瞳試験ならびに暗室試験は，特に狭隅角緑内障診断に適する負荷試験であるが，広隅角緑内障にも陽性のことがある。両試験の作用機序は多少異なり，暗室試験においては，局所性の他に中枢性の因子も関与していると考えられ（Fei—genbaum, Magitot, Weinstein, Higgit, Thiel＆Hollwich, Ungerら），事実両試験の成績間に著しい相関がない（Bloomfield＆Kellerman，真壁）ことから，散瞳，暗室試験を合併して同時に施行することにより，より高い診断的価値が期待される。

　以上の考えに基づき，合併散瞳・暗室試験を試みた。まず正常眼につき実験して，陽性判定基準を算定した。

A combined provocation test for glaucoma was developed consisting of induced mydriasis with instillation of Mydriaticum Roche followed by confinement in darkness for one hour. A to-tal of 122 normal eyes with wide chamber an-gle in 68 test subjects (36 males and 32 fe-males) were subjected to the study. The ages of the test subjects ranged from 20 to 82 years and averaged 49.0 years.

1. The intraocular pressure, which rangedfrom 10 to 22 (average : 17. 4mmHg) before the test started, showed a rise of an average of 2. 98mmHg which was judged as highly signi-ficant.
