緑内障の薬物的治療

Medical Treatment of Glaucoma.

I．緒言

　眼疾患の薬剤による治療は近年多くの新しい薬物の発見，特にサルファ剤，抗生物質と副腎皮質ホルモンの開発によつて画期的な進歩をとげた。ある意味ではこういう薬を使うことで，病気の治療は単純化してきたといつてもよい。

　ところで緑内障の治療については，最近著しく進歩したということができるが，その反面次々と新しい薬剤が出現して，治療体系が複雑化し，これらをcase by caseに使いこなすにはかなりの努力を必要とする。緑内障は40歳以上の人口の1〜2％に発見されるといわれており，その上経過の長い疾患であるため，日常の診療においても相当な比重をもつものである。新しい緑内障治療の薬剤の知識を身につけてこれを使いこなすことはわれわれの課題の一つといえる。

The present status of medical treatment of glaucoma is discussed, based on a lecture for ophthalmologists in practice held on July 12, 1966. Particular mention is made on lately developed long-term miotics (Tosmilen, Phos-pholine iodide) and on osmotic agents (hyper-tonic urea, mannitol and glycerol). Caution is needed against prolonged medical treatme-nt of chronic angle-closure glaucomas, as goni-osynechia formation is the usual sequel which results in poor prognosis in spite of favoura-ble initial clinical response. In such cases an early surgery is rather indicated.
