糖尿病の眼合併症—特に網膜症を中心に

Ocular Involvement in Diabetes Mellitus

I．はじめに

　戦時中から終戦直後にかけて一時激減したかに見えた糖尿病も，国民生活の安定と共に再び急激に増加して来た。東大小林名誉教授の日本医学会に於ける発表によれば，本邦各地で行なつた集団検診（主として40才以上の男子）の成績を集計すると昭和32〜33年に4.1％検出された糖尿病患者が，昭和37年には7.0％と増加しているという。そこで現在男女合せて全人口の5％が糖尿病患者であるとしても日本全国に480万，東京都内だけでも約50万人の糖尿病患者がいるものと推定される。

　一方糖尿病による死亡率は，昨年度の厚生省の発表によれば，人口10万に対する死亡者数がアメリカ16.4人，西ドイツ13.3人，イングランド8.4人（いずれも1961の統計）であるのに対し，日本は4.1人（1963年度統計）と著しく少く，従つて厚生省では未だ所謂成人病対策の対象としては糖尿病を考えていない。しかしこれは死因の分類法にも問題があり，後述する各種の合併症で死亡したものはいずれもその個々の合併症名の方へ加算される仕組みとなつているから，実際には死亡率も遙かに高いもので，内科関係ではすでに主要な成人病の1つと考えている。

A detailed description is given over the ocular complication in diabetes mellitus based upon public statistics and personal experi-ences of the author at the Department of Oph-thalmology of University of Tokyo. After dis-cussion on pathology, symptomatology and methods of treatment of the ocular complica-tion of the disease, grounds are given in sup-port of the view of the author that satisfac-tory systemic control of the diabetes is of primary and of utmost importance in the treatment of diabetic involvement of the eye including retinopathy.
