多発硬化症疑症例及び視束脊髄炎Devic氏病症例の開頭

Ⅰ．緒言

　球後視神経炎なるものが，視神経実質の炎症又は変性と同時に多く視交叉クモ膜病変を併せ有すると考えられ，井街等^10）は諸種中毒，レーベル氏病，感染症，ワクチン接種等に見られた球後視神経炎に対して開頭術を行い，これに視神経交叉槽クモ膜炎をみとめ，視神経周囲の肥厚，癒着せるクモ膜を剥離し，その機械的圧迫を除去し，髄液還流を助けることにより視力の改善を来すことを認めた。

　今回著者はすでに両眼視神経萎縮像を呈し，臨床的に多発硬化症と診断された1症例及びDevic氏病の1症例に対して開頭手術を行い，共に視神経交叉部クモ膜の肥厚，癒着を来し病理的にもクモ膜炎の像を認め，その前者においては視力もやや改善されたのでここに報告する。

Craniotomy was undertaken in two cases to which an initial diagnosis of multiple scle-rasis and Devic's disease had respectively been given about 3 years before. In both cases the arachnoid of the optochiasmatic region showed proliferation, thickening, adhe-sion and round cell infiltration. Removal and excoriation of the affected arachnoid resulted in slight but noticeable improvement of hype-sthesia and ataxia which were present before the operation. An improvement in visual acuity also set in one case with multiple sclerosis.
