漿液性網膜色素上皮剥離を伴った中心性輪紋状脈絡膜ジストロフィの1例

A case of central areolar choroidal dystrophy with serous pigment epithelial detachment

　家族歴のない両眼中心性輪紋状脈絡膜ジストロフィと考えられる42歳女性に，右眼視力低下と右視野の暗点拡大が生じた。右視力は矯正0.03まで低下し，中心暗点が拡大した。右眼地図状病変の周囲にある点状病巣部に，非常に薄い漿液性網膜剥離を生じていた。螢光眼底で，同部位の螢光漏出が，以前に比較し増悪していた。これは，同部位での網膜色素上皮—脈絡膜毛細管板レベルの柵破綻と考えられた。この部位への網膜光凝固により，視力は0.5に回復し，視野欠損も左眼と同程度に回復した。

A 42-year-old female had been under regular follow-up for nonhereditary central areolar choroidal dystrophy in both eyes. Her corrected visual acuity was 0.5 right and 0.7 left. Her right visual acuity gradually deteriorated to 0.2 and, 14 weeks later, to 0.03. The affected eye showed serous detachment of retinal pigment epithelium in the macular area. Fluorescein angiography showed increased dye leakage, suggesting impaired barrier at the level of retinal pigment epithelium and choriocapillaris. Photocoagulation to the site of leakage resulted in improvement of vision to 0.5 and in recovery of central scotoma to the previous state.
