孤立性網膜血管腫に続発した黄斑円孔の1例

Macular hole formation in a case of solitary retinal hemangioma

要約　目的：光凝固後に黄斑円孔が生じた孤立性網膜血管腫の症例の報告。症例：40歳女性が2か月前からの左眼視力低下で受診した。祖母と祖母の妹が原因不明の疾患で失明している。両眼の耳側に孤立性網膜血管腫があり，左眼には滲出性変化と牽引性網膜剝離があった。両眼に光凝固を行った。1年後に再び左眼視力が低下して受診した。矯正視力は右1.5，左0.3で，血管腫の大きさには変化がなく，左眼に黄斑円孔があった。硝子体手術を行い，黄斑上膜と内境界膜を剝離し，SF₆ガスを注入した。黄斑円孔は閉鎖し，視力は5か月後に0.6に改善した。結論：孤立性網膜血管腫では，光凝固後に黄斑上膜や黄斑円孔が生じることがある。

Abstract.　Purpose：To report a case of solitary retinal hemangioma who developed macular hole following photocoagulation. Case：A 40-year-old female presented with impaired vision in her left eye since 2 months before. Her grandmother and grand aunt had become blind for unknown reason. Findings：Both eyes showed solitary hemangioma in the temporal periphery. Both eyes were treated by photocoagulation. She noticed impaired vision in the left eye one year later. Corrected visual acuity was 1.5 right and 0.3 left. There was no change in the size of hemangioma in either eye. The left eye showed macular hole. She received vitreous surgery with removal of epiretinal and internal limiting membrane followed by intravitreal sulfer fluoride. Macular hole closed with visual acuity improving to 0.6 five months later. Conclusion：This case illustrates that macular hole with epiretinal membrane may develop in an eye with solitary retinal hemangioma treated by photocoagulation.
