シクロスポリン点眼が奏効した抗悪性腫瘍薬エルロチニブによる角膜潰瘍の1例

要約　目的：エルロチニブによる治療中に角膜潰瘍が生じた1例の報告。症例：73歳男性が右眼の充血と眼痛で受診した。5年前に肺腺癌の手術を受け，2年後に肺内転移が多発し，ゲフィニチブ（イレッサ^®）の内服を開始した。8か月前に角膜辺縁部潰瘍が右眼に生じ，ベタメタゾン点眼で治癒した。その1か月後からエルロチニブ内服を開始した。所見：角膜辺縁部潰瘍が右眼にあった。ベタメタゾン点眼は奏効せず，2か月後にシクロスポリン点眼に変更し，1か月後に治癒した。さらに1か月後に角膜辺縁部潰瘍が左眼に生じたが，シクロスポリン点眼で治癒した。以後14か月間に角膜病変の再発はない。結論：悪性腫瘍に対するエルロチニブ内服で角膜潰瘍が生じる可能性がある。

Abstract.　Purpose：To report a case who developed corneal ulcer during systemic treatment with erlotinib. Case：A 73-year-old male presented with hyperemia and pain in the right eye. He had received surgery for adenocarcinoma of the lung 5 years before. Pulmonary metastasis developed 2 years later followed by systemic treatment with gefinitib. Marginal corneal ulcer developed in the right eye 8 months before and cured after instillation of betamethasone. He started receiving erlotinib one month later. Findings：Marginal corneal ulcer was present in the right eye. It persisted after 2 months of treatment by topical betamethasone. It cured after treatment was switched to topical cyclosporine. He has been doing well for 14 months until present. Conclusion：Systemic treatment with erlotinib may induce corneal ulcer.
