Abstract. Purpose:To report 2 cases who developed corneal endothelial damage following long-term systemic treatment with amantadine for Parkinsonism. Cases:Two males presented with suddenly impaired vision in both eyes. They were aged 66 and 76 years respectively. Both had Parkinsonism and had been receiving amantadine for 5 and 10 years respectively. Findings:Corrected visual acuity was 0.03 in either eye in one case and 0.02 in the other. All the 4 eyes showed severe corneal edema. Signs of inflammation were absent in the anterior chamber. Discontinuation of amantadine and instillation of betamethasone was followed by disappearance of corneal edema with improved visual acuity to 0.8 or over. Population of corneal endothelial cells was below 1,000/mm2 in all the 4 eyes. Conclusion:These cases illustrate that severe damage to corneal endothelium may result after prolonged amantadine.
