要約 51歳，男性．初診半年前から臀部，下肢に暗紫紅色の皮疹が出現し潰瘍化した．初診時，鶏卵大までの扁平に隆起する局面が多発し，びらんおよび潰瘍を伴っていた．組織学的には真皮上層に好中球性膿瘍と異物型巨細胞を混じる類上皮細胞肉芽腫がみられた．プレドニゾロン内服にて軽快したが，PSL減量中に両下腿に壊死組織を伴った深い潰瘍が出現し，シクロスポリン併用にて軽快した．自験例は，通常のsuperficial granulomatous pyodermaと比較して難治性の経過をとった点が特徴的であった．
