Abstract. Purpose:To report the outcome of cataract surgery in patients with the fellow eye lost due to leprosy. Cases and Method:This retrospective study was made on 8 patients of leprosy who received phacoemulsification-aspiration with intraocular lens implantation for cataract in one eye. All had a history of uveitis and visual acuity less than hand motion in the fellow eye. Five eyes had no light perception. All the cases were followed up for 20 to 157 months,average 104 months,after surgery. Results:All the 8 eyes showed best visual acuity of 0.5 or over. Final visual acuity was better by 2 lines or more in 5 eyes. Three eyes had final visual acuity of 0.5 or over. Visual acuity less than 0.5 was due to keratopathy or macular degeneration. Conclusion:Even when the fellow eye had been lost,cataract surgery was safe and effective for the other eye in leprosy patients with uveitis. Postoperative visual acuity may deteriorate on long-term basis due to ocular lesions other than uveitis.
