Abstract. Purpose:To report an infant with gonococcal conjunctivitis resistant to new quinolones. Case:A 3-month-old male infant presented with swollen eyelids,profuse discharge and hyperemia in both eyes. The findings were suggestive of acute viral conjunctivitis. Topical treatment with moxifloxacin was ineffective. Gram-negative diplococcic were present in the discharge. The treatment was switched to topical cefmenoxime with systemic ceftriaxone and cefdinir from day 2 of the disease. Clinical symptoms improved promptly. Neisseria gonorrhoeae was isolated from cultured discharge. The strain was resistant to new quinolones. PCR studies of family members were negative throughout. Conclusion:Cases suspected of gonococcal conjunctivitis need prompt diagnosis by microscopy of discharge followed by selection of antibacterial drugs with due attention to possible drug resistance.
