生後3か月の乳児に発症したニューキノロン耐性淋菌性結膜炎の1例

Gonococcal conjunctivitis resistant to new quinolone in a three-month infant

要約　目的：ニューキノロン耐性の淋菌性結膜炎が発症した乳児の報告。症例：生後3か月の男児が両眼の眼瞼腫脹，眼脂と充血で救急受診した。ウイルス性結膜炎が疑われた。ニューキノロン薬のモキシフロキサシン点眼は無効であった。眼脂の鏡検にてグラム陰性双球菌が認められた。第2病日からセフェム系のセフメノキシム点眼，セフトリアキソン点滴，セフジニル内服を行い，病像が改善した。眼脂からNeisseria gonorrhoeaeが培養され，ニューキノロン耐性であった。家族に行った血液のPCR検査はすべて陰性であった。結論：淋菌性結膜炎が疑われる症例では，眼脂の塗抹検査による迅速診断と，薬剤耐性株による感染を考慮した抗菌薬の選択が重要である。

Abstract.　Purpose：To report an infant with gonococcal conjunctivitis resistant to new quinolones. Case：A 3-month-old male infant presented with swollen eyelids,profuse discharge and hyperemia in both eyes. The findings were suggestive of acute viral conjunctivitis. Topical treatment with moxifloxacin was ineffective. Gram-negative diplococcic were present in the discharge. The treatment was switched to topical cefmenoxime with systemic ceftriaxone and cefdinir from day 2 of the disease. Clinical symptoms improved promptly. Neisseria gonorrhoeae was isolated from cultured discharge. The strain was resistant to new quinolones. PCR studies of family members were negative throughout. Conclusion：Cases suspected of gonococcal conjunctivitis need prompt diagnosis by microscopy of discharge followed by selection of antibacterial drugs with due attention to possible drug resistance.
