Abstract. Purpose:To report a case of sarcoidosis manifesting giant iris nodules and marked reduction in CD4-positive T lymphocytes. Case:A 19-year-old female presented with photophobia since 2 months before. She had been diagnosed with sarcoidosis after detection of hilar lymphnode hypertrophy and lung biopsy 3 months before. Findings:Corrected visual acuity was 0.4 right and 0.8 left. The right eye showed signs of iritis with two giant nodules in the chamber angle. The left eye showed no abnormalities. She showed generalized swelling of lymphnodes,marked reduction in CD4-positive T lymphocytes,and was false-positive for HIV. Biopsy of lymphnodes and positron emission tomography showed findings compatible with sarcoidosis. Ocular and systemic lesions improved after 10 months of treatment with corticosteroid. Conclusion:Findings simulating malignant lymphoma in the present case proved to be due to sarcoidosis.
