要約　目的：全身サルコイドーシスに巨大虹彩結節が合併し，超音波生体顕微鏡でその経過を観察した報告。症例：33歳男性が右眼の霧視で受診した。矯正視力は左右眼とも1.2で，右眼に巨大な虹彩腫瘤と虹彩炎の所見があった。X線検査で肺門リンパ腺の腫脹があり，CTで縦隔などにリンパ腺腫大と，陰囊に腫瘤があった。生検でサルコイドーシスの診断が確定し，虹彩結節もこれによると推定された。ステロイド薬の点眼を行い，4か月後に虹彩結節は消退した。その3か月後に再発があったが，ステロイド薬の局所治療の強化で寛解した。結論：サルコイドーシスによる巨大虹彩結節の診断と治療効果の判定に，超音波生体顕微鏡が有用であった。

Abstract.　Purpose：To report giant iris nodule in a case of sarcoidosis followed up by means of ultrasound biomicroscope. Case：A 33-year-old male presented with blurring in the right eye. Corrected visual acuity was 1.2 in either eye. The right eye showed a giant iris tumor and signs of iritis. X-ray studies showed enlarged hilar lymph nodes. Computed tomography showed multiple tumors including the mediastinum. Another tumor was present in the scrotum that proved to be sarcoidosis by biopsy. Bronchoscopy also supported the diagnosis. Topical corticosteroid was followed by dissolution of iris tumor 4 months. It finally disappeared after minor recurrence lapse another 3 months later. Conclusion：Ultrasound biomicroscopy was useful in the diagnosis and follow-up in a case of giant iris tumor.
