関節症性乾癬によるぶどう膜炎にインフリキシマブ治療が奏効した1例

Systemic infliximab was effective in a case of uveitis with psoriatic arthritis

要約　目的：乾癬性ぶどう膜炎にインフリキシマブの静注が奏効した症例の報告。症例：58歳男性が3年前からの両眼ぶどう膜炎で受診した。28年前に仙腸関節炎が発症し，11年前に関節症性乾癬と診断された。所見：両眼に前部ぶどう膜炎があり，乾癬性ぶどう膜炎と診断した。以後14年間，ステロイド点眼をしても眼炎症が遷延し，シクロスポリン内服も奏効しなかった。初診から15年後に皮膚症状が悪化し，インフリキシマブの点滴静注を開始した。静注開始後すみやかに皮膚症状は改善し，眼炎症も軽快した。結論：ステロイド点眼やシクロスポリン内服で十分な効果が得られなかった乾癬性ぶどう膜炎が，インフリキシマブ点滴静注により皮膚症状とともに改善した。

Abstract.　Purpose：To report a case of psoriatic uveitis that markedly responded to intravenous infliximab. Case：A 58-year-old male presented with anterior uveitis in both eyes since 3 years before. He had had iliosacral arthritis 28 years before. He had been diagnosed with psoriatic arthritis 11 years before. Findings：Both eyes showed anterior uveitis and were diagnosed with psoriatic uveitis. During the ensuing 14 years,the uveitis persisted despite instillation of corticosteroid and systemic cyclosporine. Cutaneous manifestations exacerbated 15 years after his initial visit. Intravenous infusion of infliximab was promptly followed by improvements of skin and ocular lesions. Conclusion：The present case illustrates that systemic infliximab may induce improvements in psoriasis and uveitis.
