高知 町田病院における細菌性角膜炎の検討

Review of bacterial keratitis in our institution

要約　目的：過去6年間に経験した細菌性角膜炎の報告。対象と方法：2009年までの6年間に高知市の町田病院で入院加療した35例を検索した。結果：すべて片眼性で，男性22例，女性13例であり，24例（68％）が65歳以上であった。外傷や水疱性角膜炎などの既往が21例（60％）にあった。起炎菌は20例で同定され，グラム陽性球菌6例，グラム陰性桿菌13例，グラム陰性球菌1例であった。グラム陰性桿菌による角膜炎は，治療後に角膜浮腫が悪化し，角膜上皮欠損が遷延化する傾向があった。結論：高齢者でもグラム陰性桿菌による角膜炎が問題である。

Abstract.　Purpose：To present a review on bacterial keratitis treated at Machida Hospital in Kochi. Cases and Method：This retrospective study was made on 35 eyes of 35 cases treated during 6 years through 2009. Findings：Twenty-four cases（68％）were aged 65 years or over. Bacterial keratitis was preceded by trauma or bullous keratopathy in 21 cases（60％）. Causative organism could be identified in 20 cases,including gram-positive cocci 6 cases,gram-negative bacilli 13 cases,and gram-negative cocci one case. Keratitis due to gram-negative baccillli tended to result in exacerbation of corneal edema after treatment and in protracted course of corneal epithelial defect. Conclusion：Keratitis due to gram-negative bacilli is a problem in persons of advanced age.
