Abstract. Background:Irradiation by heavy particles,including carbon-ion,promises to be effective for malignant melanoma. Purpose:To report a case of malignant choroidal melanoma that relapsed 4 years after initially effective carbon-ion radiotherapy. Case:A 53-year-old female was diagnosed with malignant melanoma of the choroids in her left eye. The tumor was 17 mm in diameter and 12 mm in thickness. The tumor was treated by carbon-ion at the total dosis of 77 GyE. Visual acuity decreased from 0.3 to nil 6 months later due to progression of retinal detachment. Magnetic resonance imaging(MRI)showed the tumor to have decreased in size. The tumor enlarged 4 years later,leading to enucleation. The tumor was spindle-shaped malignant melanoma and was relapse from the original irradiated area. Conclusion:Carbon-ion radiation was initially effective for malignant melanoma of the choroid but relapsed 4 years later. Insufficient dosis of radiation is ruled out. This case illustrates the need for long-term follow-up in apparently controlled tumor immediately after irradiation.
