Abstract. Purpose:To report a case of acute retinal pigment epitheliitis(ARPE)associated with white spots in the fundus and choroidal neovascularization(CNV). Case:A 18-year-old woman presented with photopsia and impaired visual acuity in her right eye since 2 weeks before. Findings:Corrected visual acuity was 0.6 right and 1.2 left. Both eyes showed dark-gray spots in the macula surrounded by yellowish white halo. Visual acuity recovered 2 weeks later. Right visual acuity decreased to 0.4 four years later. Macular lesion remained unchanged but numerous white dots were present in the midperipheral fundus. She was diagnosed with ARPE. Spontaneous recovery set in 2 months later. Another 4 months later,right visual acuity decreased to 0.09. Serous retinal detachment with CNV was present adjacent to the macula. Three sessions of vitreal injection of bevacizumab was followed by improved visual acuity to 0.4 ten months later. Conclusion:While ARPE has a favorable prognosis in general,this case illustrates that CNV may develop after recurrences of ARPE.
