Abstract. Purpose:To report the influence of prolonged presence of silicone oil in the vitreous on cerebral ventricles and peribulbar tissue examined by magnetic resonance imaging(MRI). Cases and Method:This study was made on 9 eyes of 7 cases who received intravitreal injection of silicone oil for proliferative diabetic retinopathy or vitreoretinopathy. The age ranged from 34 to 74 years,average 54 years. Silicone oil had been present from 20 to 123 months,average 42 months. Results:One eye showed presence of emulsified silicone oil in the anterior chamber. There was no migration of silicone oil in the cerebral ventricles throughout. Two eyes showed presence of silicone oil in the subconjunctival space. There was no instance of migration of silicone oil into the orbital tissue. Five cases(71%)had developed cerebral infarction after vitreous surgery. Conclusion:Silicone oil did not migrate into extrabulbar tissue even after prolonged presence in the vitreous. High incidence of cerebral infarction show the importance of systemic management after vitreous surgery.
