16歳男性に発症した網膜中心静脈閉塞症の1例

Central retinal vein occlusion in a male sixteen years of age

要約　目的：16歳の男性に発症した網膜中心静脈閉塞症の報告。症例：16歳男性が1週間前からの右眼視力低下で受診した。所見：矯正視力は右0.5，左1.5で，右眼に乳頭発赤と静脈蛇行を伴う網膜出血があった。血液所見は白血球の増加以外はすべて正常であった。蛍光眼底造影では血管床の閉塞や囊胞様黄斑浮腫はなかった。アスピリンとプレドニゾロン内服で1か月後に眼底所見は改善し，右眼視力が1.2に回復した。結論：本症例は乳頭血管炎に続発した非虚血性の網膜中心静脈閉塞症であると推定される。

Abstract.　Purpose：To report a juvenile case of central retinal vein occlusion. Case：A 16-year-old male presented with impaired visual acuity in the right eye since one week before. Findings：Corrected visual acuity was 0.5 right and 1.5 left. The right eye showed retinal hemorrhage associated with hyperemia of optic disc and tortuous veins. Hematological findings were within normal limits except leukocytosis. Fluorescein angiography showed no occlusion of vascular bed or cystoid macular edema. He was treated by peroral aspirin and prednisolone. Fundus findings improved one month later with visual acuity of 1.2 right. Conclusion：There is a high possibility that nonischemic central retinal vein in this patient was secondary to optic disc vasculitis.
