若年者に発症した両眼の網膜静脈閉塞症の1例

Bilateral central retinal vein occlusion in a young adult

　25歳女性が右眼視力低下を主訴として受診した。気管支喘息があり，少なくとも過去10年以上，副腎皮質ステロイド薬を内服していた。矯正視力は右0.15，左0.8であった。右眼に網膜中心静脈閉塞症，左眼に切迫型網膜中心静脈閉塞症の所見があった。左眼はいったん寛解したが，初診から5年後に視力が0.2に低下した。半側（hemi）網膜中心静脈閉塞症の所見があった。経過中に高血圧，糖尿病はなく，血液一般検査所見は正常であり，抗リン脂質抗体症候群あるいは高粘稠症候群は否定された。このような若年成人の両眼に網膜中心静脈閉塞症が発症したことは異例なことであり，長期間の副腎皮質ステロイド薬の内服で血栓形成が誘発された可能性がある。

Purpose：To report a young patient with recurrent bilateral central retinal vein occlusion（CRVO）. Design：Observational case report. Case：A 25-year-old female presented with decreased vision in her right eye. She had been treated with peroral corticosteroid for bronchial asthma in the past 10 years. Corrected visual acuity was 0.15 right and 0.8 left. Funduscopy showed CRVO in the right eye and impending CRVO in the left. After spontaneous resolution,her left eye developed hemi-CRVO 5 years later. Throughout the course of the disease,she was free of systemic hypertension,or diabetes mellitus and manifested normal hematological findings. We ruled out antiphospholipid antibody syndrome or hyperviscosity syndrome. Conclusion：There is a high possibility that prolonged systemic treatment with corticosteroid faciliated formation of thrombosis that resulted in bilateral CRVO.
