小児急性リンパ性白血病に対する骨髄移植後の完全寛解期に起こった眼内再発

Intraocular recurrence in a child after complete remission of acute lymphoblastic leukemia

要約　目的：小児急性リンパ性白血病の完全寛解期に眼内再発が起こった症例の報告。症例：7歳の男児が左眼網膜剝離として紹介され受診した。5歳時に急性リンパ性白血病と診断され，6歳時に同胞から骨髄移植を受け，完全に寛解した。所見：矯正視力は右1.5，左手動弁で，左眼に硝子体混濁，網膜全剝離，乳頭腫脹があった。1か月後に眼圧が上昇し，新生血管緑内障が生じた。化学療法と放射線照射で眼圧は正常化したが視力は失われ，眼球癆になった。受診から2年後に右聴神経，動眼神経，三叉神経などに腫瘍の浸潤が生じ，全脳照射ののち再度の骨髄移植を行った。以後3か月後の現在まで頭蓋内病変と全身状態は良好である。結論：小児急性リンパ性白血病は治癒率と生存率が高いが，完全寛解した後でも眼領域に再発することを本症例は示している。

Abstract.　Purpose：To report a child who developed ocular relapse after apparent complete cure of acute lymphoblastic leukemia. Case：A 7-year-old boy was referred to us for retinal detachment in the left eye. He had been diagnosed with acute lymphoblastic leukemia at 5 years old that subsided after bone marrow transplantation 11 months later. Findings：Visual acuity was 1.5 right and hand motion left. The left eye showed vitreous opacity,swollen optic disc,and total retinal detachment. Neovascular glaucoma developed 1 month later. It subsided after radiation to the brain and chemotherapy but the left eye became phthisic. Two years after his initial visit,infiltration of tumor occurred in the oculomotor,trigeminal and acoustic nerves. He is doing well for 3 months until present after radiation and second bone marrow transplantation. Conclusion：This case illustrates that ocular relapse is a possibility even after complete remission of childhood acute lymphoblastic leukemia.
