網膜芽細胞腫に対する治療成績

Outcome of treatment for retinoblastoma

要約　目的：網膜芽細胞腫に対する治療成績の報告と国際分類（2003）の評価。対象と方法：2008年までの7年間に網膜芽細胞腫21例29眼が受診した。初診時の年齢は1～106か月（平均18か月）で，男児13例（62％），女児8例（38％）であり，13例が片眼，8例が両眼発症であった。初回治療として眼球を摘出しなかった24眼に，化学療法と経瞳孔温熱療法による保存治療を行った。国際分類による24眼の内訳は，A群0眼，B群11眼，C群1眼，D群9眼，E群3眼であった。結果：24眼のうち眼球が温存できたのは，B群11眼（100％），C群1眼（100％），D群2眼（22％），E群0眼であった。結論：網膜芽細胞腫に対する化学療法を主とする治療成績は，国際分類をよく反映している。

Abstract.　Purpose：To report the outcome of conservative treatment for retinoblastoma and to evaluate the International Classification of Retinoblastoma（2003）. Cases and Method：A total of 29 eyes of 21 patients were diagnosed with retinoblastoma during 7 years through 2008. The series comprised 13 male and 8 female infants. Thirteen cases were unilaterally and 8 cases were bilaterally involved. The age at the initial visit ranged from 1 to 106 months,average 18 months. Enucleation had to be performed on 5 eyes as initial treatment. The other 24 eyes received chemotherapy and transpupillary thermotherapy. The 24 eyes comprised stage B 11 eyes,stage C 1 eye,stage D 9 eyes,and stage E 3 eyes. Results：The eyeglobe could be preserved in 11 eyes of stage B（100％）,1 eye of stage C（100％）,2 eyes of stage D（22％）,and none of stage E. Conclusion：The outcome of treatment in the present cases is well in accord with the International Classification of Retinoblastoma.
